Einem Verlust von physischer und psychischer Leistungsf\u00e4higkeit soll rechtzeitig vorgebeugt werden.<br \/>Damit m\u00f6chte <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong> diese Zielgruppe erreichen, bevor die Schmerzen chronifiziert sind, sich Arbeitsunf\u00e4higkeit und weitere biopsychosozialen Einschr\u00e4nkungen eingestellt haben. <br \/>Die Unterversorgung in der Behandlung von Betroffenen mit Schmerzen im Sinne eines pr\u00e4ventiven Ansatzes soll endlich geschlossen werden. Die Konzeption sieht ein Gruppenkonzept mit erg\u00e4nzenden Einzelsitzungen vor. Eine regelm\u00e4\u00dfige Teamsitzung f\u00fchrt die Informationen aus den Gruppen- und Einzelkontakten zusammen und erm\u00f6glicht eine integrative Sicht f\u00fcr Diagnosestellung, Durchf\u00fchrung der Interventionen und abschlie\u00dfenden weiterf\u00fchrenden Empfehlungen.<br \/>Entsprechend aktueller Literatur dient eine Mischung aus edukativen Anteilen und aktiven \u00dcbungen einer Vermeidung weiterer Chronifizierung von Schmerz.<br \/>In <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong> sollen dabei sowohl \u00e4rztliche als auch psychologische und physiotherapeutische KollegInnen interdisziplin\u00e4r zusammenarbeiten. Innovativ ist der Ansatz zum einen, weil interdisziplin\u00e4re Ans\u00e4tze in der Pr\u00e4vention noch nicht weit verbreitet sind. Zum anderen arbeiten die Professionen auch eng in der Gruppendurchf\u00fchrung zusammen, d.h., dass die Gruppe durch mindestens zwei Professionen gleichzeitig betreut wird. Damit soll ein guter Wissenstransfer und die Vermittlung des biopsychosozialen Modells aus allen verf\u00fcgbaren Perspektiven erreicht werden.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div aria-labelledby=\"elementor-tab-title-1755\"><p><span style=\"font-weight: 400;\">Aufgenommen werden PatientInnen aller gesetzlichen Krankenversicherungen, die <\/span><b>wiederkehrende Schmerzen von mehr als 6 Wochen oder wiederholt auftretende und anhaltende Schmerzen haben<\/b><span style=\"font-weight: 400;\">, wenn sie:<\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">mindestens 18 Jahre sind,<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">sich in ihrer Lebensf\u00fchrung (Alltag, Beruf, Familie und\/oder Freizeit) durch diese Schmerzen eingeschr\u00e4nkt f\u00fchlen,<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">ggf. bereits aufgrund der Schmerzen arbeitsunf\u00e4hig sind <\/span><i><span style=\"font-weight: 400;\">und<\/span><\/i><\/li><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">erste Anzeichen auf Risikofaktoren f\u00fcr eine Schmerzchronifizierung aufweisen.<\/span><\/li><\/ul><p><span style=\"font-weight: 400;\">Die PatientInnen m\u00fcssen \u00fcber ausreichend Kenntnisse der deutschen Sprache in Wort und Schrift verf\u00fcgen und sollten in erreichbarer Entfernung zum teilnehmenden <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0 <\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">-Zentrum wohnen. <\/span><\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-accordion-item\">\n\t\t\t\t\t<div Ist dies der Fall, wird das Vorliegen von Ausschlusskriterien f\u00fcr <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">\u00a0gepr\u00fcft. Dazu z\u00e4hlen unter anderem:<\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">klinische Hinweise f\u00fcr eine schwerwiegende, dringend akut therapiebed\u00fcrftige Erkrankung (z.B. \u201eRed Flags),<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">andere, akut wirksame schwerwiegende Erkrankungen, die z.B. eine aktivierende Behandlung verhindern (Bsp. Schwere Herzinsuffizienz, Tumor),<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">eine bereits eingetretene manifeste chronische Schmerzerkrankung (z.B. AU wg. Schmerzen l\u00e4nger als 6 Monate, vorhergehende Therapie mit starken Opioiden \u00fcber 3 Monate Dauer, vorhergehende IMST in letzten beiden Jahren),<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">eine schwere und aktive psychiatrische St\u00f6rung (Pers\u00f6nlichkeitsst\u00f6rung, schwere Depression, Hinweise auf Suizidalit\u00e4t),<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">ein laufendes Rentenverfahren <\/span><i><span style=\"font-weight: 400;\">oder<\/span><\/i><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">sprachliche u.\/o. kognitive Einschr\u00e4nkungen.<\/span><\/li><\/ul><p><span style=\"font-weight: 400;\">Sollten PatientInnen eines der Ausschlusskriterien aufweisen, k\u00f6nnen sie nicht an <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\"> teilnehmen. Der Teledoktor f\u00fchrt ein kurzes Screening-Interview durch und informiert die PatientInnen \u00fcber <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">. Sind die PatientInnen allgemein geeignet und an einer Studienteilnahme interessiert, versucht der Teledoktor die PatientInnen telefonisch direkt an ein <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">-Zentrum durchzustellen. Falls keine direkte telefonische Vermittlung m\u00f6glich ist, erh\u00e4lt der\/die PatientIn die Kontaktdaten des Zentrums. Dann kann telefonisch ein Termin f\u00fcr einen Informationstermin vereinbart werden, bei dem ein\/e Arzt\/\u00c4rztin die Ein- und Ausschlusskriterien\u00a0genauer pr\u00fcft und \u00fcber einen Studieneinschluss entscheidet.<\/span><\/p><p><b>2.) Anmeldung \u00fcber einen kooperierende\/n Arzt\/\u00c4rztin<\/b><\/p><p><span style=\"font-weight: 400;\">Sie als kooperierende\/r Arzt\/\u00c4rztin (z.B. Hausarzt\/-\u00e4rztin, Betriebsarzt\/-\u00e4rztin, Orthop\u00e4de\/-in) identifizieren potentielle PatientInnen (aller teilnehmenden Krankenkassen \u2013 <strong><span class=\"purple\">BARMER<\/span><\/strong><\/span><b>\u00a0<\/b><span style=\"font-weight: 400;\">und andere) und pr\u00fcfen ggf. die Ein- und Ausschlusskriterien anhand des Aufnahmebogens (<\/span><b>B5<\/b><span style=\"font-weight: 400;\">) und des Patientenfragebogens FB-S (<\/span><b>B4<\/b><span style=\"font-weight: 400;\">). Geeignete PatientInnen k\u00f6nnen Sie an das regionale <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">-Zentrum verweisen, indem Sie den PatientInnen den Flyer mit Informationen \u00fcber <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\"> und den Kontaktdaten der Einrichtung aush\u00e4ndigen. Die PatientInnen melden sich daraufhin in der Einrichtung. Im <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">-Zentrum wird dann ein zeitnaher Informationstermin vereinbart werden. Bitte leiten Sie vorhandene Vorbefunde und ggf. Screening-Dokumente (B4 und B5) per Fax, E-Mail oder Post an das regionale <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">-Zentrum weiter.<\/span><\/p><p><b>3.) Direkte Meldung im Zentrum<\/b><\/p><p><span style=\"font-weight: 400;\">PatientInnen k\u00f6nnen sich auch direkt\/selbst im <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">-Zentrum melden. Bei (telefonischer oder pers\u00f6nlicher) Kontaktaufnahme im Zentrum, werden die Ein- und Ausschlusskriterien (z.B. Erreichbarkeit des <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">-Zentrums) gepr\u00fcft. Der Weg ohne Screening<\/b><\/p><p><span style=\"font-weight: 400;\">Sie senden PatientInnen mit folgenden Kriterien an das <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">-Zentrum:<\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">mindestens 18 Jahre sind,<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">sich in ihrer Lebensf\u00fchrung (Alltag, Beruf, Familie und\/oder Freizeit) durch diese Schmerzen eingeschr\u00e4nkt f\u00fchlen,<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">ggf. bereits aufgrund der Schmerzen arbeitsunf\u00e4hig sind <\/span><i><span style=\"font-weight: 400;\">und<\/span><\/i><\/li><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">erste Anzeichen auf Risikofaktoren f\u00fcr eine Schmerzchronifizierung aufweisen.<\/span><\/li><\/ul><p><span style=\"font-weight: 400;\">Das eigentliche Screening wird dann in dem <strong><span class=\"purple\">PAIN<span class=\"orange\">2.0<\/span><\/span><\/strong><\/span><span style=\"font-weight: 400;\">-Zentrum durchgef\u00fchrt. Beim Informationstermin werden die Ein- und Ausschlusskriterien gepr\u00fcft und die PatientInnen \u00fcber die Studie aufgekl\u00e4rt.
